APPOINTMENT DETAILS

Appointments must be made for all services.

• For an appointment at our Subiaco Clinic please call us on (08) 9382 3969. For an appointment at our South Perth Clinic please call us on (08) 9368 4560.
• Referrals are not required except for insurance claims such as workers compensation or motor vehicle accidents.

• When booking please ask for a therapist who is registered with your health fund. (Please contact your Insurer to check if you are covered).
• All accounts must be settled at the time of appointment. No accounts will be issued unless through prior arrangement with management.
• Eftpos, Visa, Bankcard, Mastercard and American Express facilities are available. We also accept cash and cheques.
• Discounts are available for pensioners, students, Triathlon WA members, SAS, WAIS and AIS athletes.
• Rebates are available from most private health insurance companies. Please ask for a receipt at the time of payment with a provider number to claim from your health insurance.
• 24 hour notice is required for cancellations. Should you fail to attend an appointment or notify us of a cancellation within 24 hours you may incur a fee.
• Gift vouchers are available for sale at both clinics.
